To help meet the medical, behavioral health and social needs of Indiana’s diverse population, Anthem Blue Cross and Blue Shield (Anthem) provides benefits for our Hoosier Healthwise, Healthy Indiana Plan (HIP) and Hoosier Care Connect members.

Hoosier Healthwise

Hoosier Healthwise is Indiana’s healthcare program for children, pregnant women and low-income families. This program for children and pregnant women includes two different benefit packages:

  • Package A is for children and pregnant women.
  • Package C is for preventive, primary and acute care services for children under 19 years of age who don’t qualify for Package A.

Healthy Indiana Plan

The Healthy Indiana Plan (HIP) was created by the Indiana State Legislature to help fund healthcare benefits for those adults who have traditionally been uninsured. HIP is for individuals aged 19 to 64. With variations, there are four base plans:

  • HIP Plus
  • HIP Basic
  • HIP State Plan Benefit
  • HIP Maternity offers the same benefits as Hoosier Healthwise Package A for pregnant women

Hoosier Care Connect

Hoosier Care Connect is the state’s program for Medicaid-eligible children and adults who have a disability, are blind or are aged 65 or older. Children in foster care, young adults transitioning from foster care, foster care alumni and children receiving adoption assistance may also be eligible. Hoosier Care Connect members cannot be enrolled in Medicare or in a home and community-based services (HCBS) waiver program or reside in an institutional setting.

Hoosier Care Connect requires coordinated, person-centered care across the delivery system and care continuum with the goal of continued improvement of coordination of care to assist providers in improving quality of care and health outcomes, including improved clinical and functional status, enhanced quality of life, improved member safety, enhanced member autonomy and adherence to treatment plans.
